Supplier Representations and Certifications
Required by Federal Procurement Regulations, suppliers must complete the applicable supplier representations and certifications form and send it to an ASRC procurement representative—by mail or fax. The ASRC Procurement representative will notify the supplier as to the applicability in completing these forms.

Supplier Annual Representations and Certifications: Required for all Purchase Agreements under a U.S. Government Prime Contract exceeding $10,000. (Forms are only required to be completed annually)
Gov - 21, (rev. March 2004)
Supplier Representations and Certifications: Suppliers must complete this form prior to Award of Purchase Agreements under a U.S. Government Prime Contract exceeding $100,000.
